This form must be completed in order to apply for the Common Housing Register ran by all of the Durham Key Options partners.

If your medical conditions have changed since you first applied and you wish to have them reassessed you should fill in this form

Applicants who cannot afford their current accommodation costs can qualify for priority status within the Durham Key Options Scheme by filling out this form.

You must complete this form if there have been any changes to your circumstances since completing your Durham Key Options Housing Application.
