This form must be completed in order to apply for the Common Housing Register ran by all of the Durham Key Options partners. |

This form allows us to assess your individual medical needs and priorities. A medical condition can affect the priority banding you are given for your application. If you have any physical or mental disabilities that need to be taken into account it is advisable to fill out this form. |

Applicants who cannot afford their current accommodation costs can qualify for priority status within the Durham Key Options Scheme by filling out this form. |

You must complete this form if there have been any changes to your circumstances since completing your Durham Key Options Housing Application. |
