Updating Applications

Applicants are obliged to notify the Durham Key Options partner organisation of any change in their circumstances which may affect their application. Changes in circumstances may affect an applicant’s banding priority. This will also result in a change to their application/ priority date which is used in the shortlisting process.

In cases where an applicant moves into a higher band, the new date for shortlisting purposes is the date that Durham Key Options were made aware of that change of circumstances. In cases where an applicant moves to a lower band, the original application date will be used. Durham Key Options will carry out an annual review of all applications, to ensure applicant’s details are accurate.

There are three forms that can be filled in to update your medical conditions, these are:

Medical Assessment Form

Download the Medical FormThis form allows us to assess any changes in your individual medical needs and priorities since you first applied/ re-registered. A medical condition can affect the priority banding you are given for your application. If you have any physical or mental disabilities that need to be taken into account it is advisable to fill out this form.

The assessment will be assessed in accordance with the information provided on the medical form, please complete all of the relevant sections. Please ensure that you give all information that you feel is relevant as only the information you provide will be used in our assessment. This may include letters from your GP or specialist for example.

Derwentside Homes will only contact your Doctor/ Hospital Consultant if further information is required. By signing the Medical form you are giving Derwentside Homes your consent to do so.

A Doctor’s letter does not guarantee any extra priority to your registration.

Hardship Assessment Form

Download the Hardship Assessment FormApplicants who cannot afford their current accommodation costs can qualify for priority status within the Durham Key Options Scheme. To help us to carry out an assessment of your financial need, please complete the Hardship Assessment Form as fully as you can, giving details of your monthly income and expenditure, and provide written proof of your circumstances where requested.

If you need help in completing this form, or accessing the scheme, please contact us and tell us what we can do to help. In all cases we will require copies of your last two Bank/Building Society statements. Any original documentation that you provide will be returned to you.

Change of Circumstances Form

Download the Change of Circumstances FormYou must complete this form if there have been any changes to your circumstances since completing your Durham Key Options Housing Application. The reasons for filling in this form may include:

  • Change of address
  • Change of persons on application
  • Change in reason for needing move
  • Any other minor changes
